I am purchasing 1 hour Clinical Hypnotherapy, Psychotherapy, Counselling supervision with Dr Tracie O’Keefe DCH at cost of $165 AUD. 

I am paying for the session now and will telephone Dr O’Keefe’s practice to book the time of the session. 

I understand I have elected to undergo supervision by Zoom or similar internet facilities.

I agree beyond this point the booking is confirmed and there is no cancellation refund due.
